Healthy Connections Service now open to receive referrals

The Healthy Connections Service (NHS social prescribing) is accepting new referrals again, having been a much reduced service between March and June.

The service is pleased to be able to offer telephone and Near Me consultations to support:

  • Making Connections to reduce loneliness and isolation.
  • Coping With Life’s Stresses by developing coping strategies and motivation to make changes, such as looking after emotional wellbeing or living well with long-term conditions and managing symptoms.
  • Daily Life: Making positive, sustainable changes like eating well, active living, alcohol, smoking or self-care.
  • Future Planning and Managing Changes in Circumstances which may be for a whole range of situations, such as change in employment circumstances, taking on a caring role or bereavement support. Links to relevant agencies can be made if desired.
